How these villain names are built
Villains in fiction share a sound: humming m and v onsets, dark o and u vowels, and endings that close on th, r or x. This generator builds two or three syllables from that inventory and keeps them pronounceable, because a name the players can say is a name they will fear.
Endings carry a gloss: -oth (the hollow), -us (the usurper), -ar (the cruel), -mor (of the black hall) and -ax (the breaker) read masculine; -a (the pale), -ith (the whisper), -ess (the sovereign), -ra (the red) and -ine (the poisoner) read feminine; -ek (the unnamed) is shared.
Titles tell you where the villain sits (Lord, Magister, High Inquisitor, the Dread Sovereign); epithets tell you what they have done (the Unmaker, the Many-Faced, of the Last Door, the Kindly). Each epithet carries a hint you can build a campaign around.

Keep the given name short and the epithet specific. Vorath the Unmaker works; Vorathaxamor the Extremely Evil does not. Set Length to Medium and pick an epithet whose hint suggests a plan rather than a mood.
Can I get a female villain?
Yes. Set Gender to Feminine for endings such as -ess (the sovereign) and -ine (the poisoner); titles switch to Lady and the Baroness.
