How these half-orc names are built

A half-orc name is a negotiation. This generator softens the orcish grammar (more open syllables, front vowels allowed, lighter clusters at the seams) so the given name still reads as orcish but a human innkeeper could say it, and it pairs that with a human surname of the plain, descriptive kind that outsiders tend to be given: Ashbrook, Stoneward, Greymoor.

Should a half-orc use an orcish or a human name?

That is the character question, and the two styles here let you answer it either way. Raised among humans: orcish given name (from the parent) plus a human surname (from the town). Raised among orcs: given name plus epithet.

Why are the surnames so plain?

Because that is how outsiders historically acquired surnames: from where they lived or what they looked like. A half-orc named Ashbrook probably lived by the ash trees at the brook, and the name says so.
